Welcome and thank you for choosing our medical practice. We want to do everything we can to address your health care needs and make your first visit comfortable and convenient. You will need to complete a number of forms prior to your first visit in order for us to get an accurate assessment of your health concerns. Clicking on either the Female or Male link will take you to a page with the forms that must be completed.

If you’re a current patient of the practice, please click on Established Patient to update your medical history. When the paperwork is complete, please fax them to (904) 249-2047 or email them to newpatient@cwrandolph.com or you can bring them with you to your appointment. Please arrive 30 minutes early so we may process the paperwork.

For your appointment, please bring:
- Insurance card
- Driver’s license
- Any necessary referrals
- Completed paperwork, if not sent in advance
